Sports Physicals
For 2024-25 the MHSAA is requiring that a sports physical is required for all students who participate in sports. A physical examinaation must be completed on or after April 15, 2024 to be considered a valid physical. The entire side of the physical that is signed by the Medical Professional must be completed. Please include the DATE OF THE EXAM ( Top right side of the MHSAA Physical Form) and the Date that the medical professional actually signs the document. If these are not completed, the physical will not be accepted.

ALL student-athletes and parent/guardians must complete all areas within FinalForms, as well as signatures for each area. Student-athletes WILL NOT be permitted to tryout or practice for any sport until all areas are complete within FinalForms.
