Sports Physicals
For 2023-24 the MHSAA is requiring that a sports physical is requird for all students who participate in sports. A physical examinaation must be completed on or after April 15, 2023 to be considered a valid physical.
Sports Physical 2023-24 - for all students who will be participating and trying out for any sport.
ALL student-athletes and parent/guardians must complete all areas within FinalForms, as well as signatures for each area. Student-athletes WILL NOT be permitted to tryout or practice for any sport until all areas are complete within FinalForms.
