For your convenience, we have included all the forms you will need to print to complete the application process for enrollment at White Pine Middle School.

Please bring your completed forms, along with all other required documentation, to the main office at White Pine.  A meeting will be arranged with the appropriate grade-level counselor to complete a schedule for your child.

From mid-June to mid-August, please call the White Pine Middle School Office prior to bringing in enrollment forms to make sure the office is open.  Thank you.
